WHO HAS TO HAVE A PHYSICAL?
Everyone in band will be required to have a physical for the 2021-22 school year.
WHERE DO I GET A PHYSICAL?
Students can receive a physical from their doctor or medical facility of choice. Some students already completed their physical by participating in the school program in May.
HOW DO I PROVIDE NCISD WITH PROOF OF THE PHYSICAL?
You need to have the 'cleared' physical form filled out by your doctor. You can get a the required physical form by clinking the link below:
