Physical and required documentation

Students participating in marching band are required to have a physical for safety.  The physical form to be filled out by a physician is available by clicking  here.  In addition to a physical, parents need to complete additional forms required by NCISD that are located on the Rank One website.  Please click the link below to complete these additional forms:

Information for Physicals

WHO HAS TO HAVE A PHYSICAL?
Everyone in band will be required to have a physical for the 2025-26 school year.

WHERE DO I GET A PHYSICAL?
The school will be offering physicals for $20 on May 2nd.  Students can also receive a physical from their doctor or medical facility of choice.  Again, all students will need to be cleared by their doctor before our first practice date in late July.  Please use the form below to use at your doctor's office.
